"Across the flats" at the top of the shaft is small enough that any wrench I have would flex instead of hold, so I've used a small bench vise (not mounted on anything) snugged down on it and held in one hand while I tightened the nut. Works great. Torqued to one medium grunt (about what it took to remove it the first time).
The shock didn't have a flat spot to hold, but I just bit the bullet and put some channel locks on the piston and torqued the upper nut to spec. It worked with only a slight scratch.
