I've been trying for days to join the FB group. I've filled out the questionaire multiple times.

Is there any way to get in touch with the Admins of that group to figure out what's going wrong?

As it seems with a number of things, if I were you I'd contact the office and provide them the details...to make sure you're checking all the boxes on your end.
